Privacy policy

Who we are
Monograph builds and maintains websites for Canadian health care providers. We are based in Vancouver, BC. Questions about this policy go to [email protected]. Our privacy officer is Arash Pourzare.
What we collect
This site collects personal information only when you choose to send it through a form. The contact form and the Practice Website Check form collect your name, email address, practice name and city, and, if you choose, a phone number, a website address, and a free text message. We ask you not to include any patient information, and we do not collect health information on this site.
When you submit a form, we also record where your visit came from: the page you landed on, the referring site if any, and campaign parameters in the address, so we can tell which channels lead to enquiries. This is stored in your browser for 90 days and sent with your form only if you submit one.
Separately, the site keeps anonymous counts of page views by page, day, and referring site. These counts contain no identifiers: no IP address, no device identifier, no cookie. They cannot be linked to you.
We do not use third party analytics or advertising trackers. We do not set cookies for tracking.
Why we collect it
To reply to your enquiry, to run the website check you requested, and to understand which sources bring enquiries. That is all.
Where it is stored and who processes it
Form submissions are emailed to us through Resend, a transactional email provider, and stored in a database hosted by Cloudflare, which also hosts this site. Both are third party processors and may store data outside Canada, including in the United States. Cloudflare also keeps standard server logs for security and operations. We do not sell personal information and we do not share it with anyone else.
How long we keep it
Enquiries are kept while we are in contact with you and for a reasonable period after, so we can respond to follow up. You can ask us to delete an enquiry at any time. Anonymous page counts are kept indefinitely because they identify no one.
Your rights
You can ask what personal information we hold about you, ask us to correct it, or ask us to delete it, by emailing [email protected]. We respond within 30 days. You can also complain to the Office of the Privacy Commissioner of Canada or, in British Columbia, the Office of the Information and Privacy Commissioner for BC.
Law that applies
The Personal Information Protection and Electronic Documents Act (PIPEDA) and British Columbia's Personal Information Protection Act (PIPA). This site is intended for health care providers in Canada.
Changes
When this policy changes, the date at the top changes with it. Material changes are noted here.
